As a member of the nursing and care management team, provides social work services that facilitate the screening of new family homes and participants; assists in matching and placement services to participants. Bachelor’s degree required.
Social worker license from the Massachusetts Board of Registration in Social Work. Minimum 2 years prior experience working with elderly or disabled adults. Bachelor’s degree plus 2 years clinical experience in the care of elderly or disabled individuals may be substituted for the above requirements. Valid driver’s license in the state of Massachusetts required.
