Nonotuck Resource Associates, values-based organization that is rapidly expanding statewide, providing Adult Family Care and Shared Living services, is searching for a Care Manager to manage our Worcester area services. This position is full-time, exempt.
Responsibilities include direct case management, participation in care coordination via the treatment team and supervision of Shared Living/Adult Family Care homes. A clear and comprehensive understanding of Adult Family Care, DDS/ABI and other community/ home care delivery services preferred. The ability to adapt to changes, modify and redirect processes is required.
Required: a minimum of Bachelor’s Degree from an accredited school, Masters preferred. At least two years recent experience in the direct care of the elderly or disabled adults is required.
