Case Manager - Per Diem - Bay Cove Human Services - Roxbury, MA

Salary: $18.27/hour

Responsibilities:

  • Community Crisis Stabilization (CCS) Case Manager provides direct services to adults at a CCS unit by assisting persons served in meeting their individual goals during their admission at the CCS.
  • S/he will carry a case load based on daily census and will provide direct care support including admissions, treatment planning, running groups, case management, and discharge.
  • CCS Case Manager contributes to Bay Cove’s mission by partnering with Person Served to work towards personal goals in order to achieve greater independence and improved quality of life.
  • Knowledgeably incorporates person centered and evidence based practices and philosophies into engagement strategies, treatment planning and the selection of interventions based on each Persons’ assessed readiness and expressed interest in order to support long-term recovery.
  • Utilize recovery-oriented language and principles in all communication and interactions with Persons, colleagues, and collaterals Ensure the unit follows the daily schedule of program activities, client meetings, and groups.
  • Oversight of therapeutic groups as leader, co-leader, and observer.
  • Complete and document admissions, treatment plans, client daily documentation, and discharge summaries in accordance with Bay Cove standards.
  • Ensure the safety of all clients by proactively addressing escalating behaviors, by providing as needed 1:1 supervision and by conducting and documenting checks,.
  • Assist with completing, faxing, and following up with referrals to providers, programs, and other levels of care including scheduling aftercare appointments with outpatient providers.
  • Assist in maintaining the cleanliness and upkeep of the unit, in meal preparation and cleanup, and in completing room set up as needed.

Qualifications:

  • At least two years direct care experience providing services to adults with mental health issues or a related population.
  • Bachelor’s Degree in Psychology, Social Work, or a related area required (work experience will be considered and may be substituted for degree) Proficient writing and verbal communications skills.
  • Excellent personal organization, attention to detail, and time management skills.
  • Proficiency in word processing and other computer functions using Windows, Word, and Google Email and Apps.
  • Ability to go up and down stairs, lift up to 20 pounds and to physical capacity to assist people with mobility impairments as needed.
