Home & Community Connections is currently seeking a values-centered clinician to work with individuals with developmental disabilities to identify behavioral support needs and how best to address them. Supported by an expert consulting Clinician, the Home & Community Connections Clinician develops and maintains behavior and medication plans, guides related data tracking, and provides effective training for staff teams and individuals supported. In addition, this person communicates and coordinates supports with the Department of Developmental Services, families of individuals supported and related care providers to ensure best practices in behavioral support and effective team communication. This position is a combination of training and support, coupled with administrative oversight.

EDUCATION REQUIREMENTS: LICSW or LMHC

EXPERIENCE REQUIREMENTS: Minimum of Two (2) years of experience in the Clinical field and demonstrated commitment to individuals who have experienced barriers, challenges and behavior related struggles that are life impacting. Bilingual/Bicultural Latinx with ability to provide support in Spanish preferred.
