LPN - Delta Projects - North Attleboro, MA

Delta Projects is hiring for an LPN in North Attleboro to assist the RN in the overall coordination of health services for individuals with intellectual and developmental and/or psychiatric disabilities living in Delta Projects residential homes. The LPN promotes the physical and behavioral health of residents by following the Standards of Nursing Practice. This is done as a member of the interdisciplinary treatment team and has the goal of assisting individuals to maximize their own health and wellness. The nurse coordinates and monitors health, wellness and medical services in accord with the requirements of regulatory agencies and in response to individualized needs.

The LPN will:

Provides, trains, and documents health care services in accordance with appropriate state and funding source regulations, nursing standards, and Massachusetts LPN license requirements, and professionally abides by all organization policies and procedures.
Functions as an integral member of an interdisciplinary team for a home by being on-site and available to the manager and direct support staff.
Collaborates with various team members to assess, organize, update, and maintain individual health care, medication and wellness needs.
Ensures all regular, specialty and follow-up care needs are met and supports the Residential Manager as needed.
Attends health care related appointments as needed.
Acts as the liaison with an individual’s RN to train and monitor the implementation of Health Care Guidelines and Protocols.
Conducts Self-Medication Assessments as part of an individual’s ISP.
Trains and monitors self-medication plans for those who pass the Self-Medication Assessment.
Provides support to the Residential Manager on the development of wellness plans including healthy diets and exercise. Provides on-site education to individuals and/or staff on wellness issues.
Participates in the admission of new individuals: assists in the accurate set-up of medication administration systems, confirms all health care guidelines and protocols are up-to-date and trained.
Works collaboratively with the RN and Residential Manager to facilitate discharges from health care facilities.
Acts as a liaison as needed to other community agencies and treatment providers regarding regular health care needs. Assists staff to identify appropriate community health resources.
Provides training and support to DPH Medication Administration Program (MAP) Certified Staff to ensure their competency in administering medications.
Provides quality assurance reviews of medication and health care systems and MAP compliance per internal QE processes.
Provide direct service to individuals as necessary and when conditions are warranted.
Conduct research for improving the nursing practices and healthcare outcomes.
Any additional duties as assigned by supervisor.

Graduate of an accredited school of Nursing
Current and valid LPN license in the state of Massachusetts
Current AHA Healthcare Provider training (BCLS) or equivalent
Current and valid driver’s license in the state of residence
Qualifying criminal background and driving record
Professional oral and written communication skills
Proficiency with Microsoft Office
