Develop, supervise and manage the provision of home-based supports and community resources for individuals with acquired brain injuries, utilizing a team-based approach
Qualifications:
- Bachelor’s degree in a human services or related field required
- CBIS Certification or commensurate experience in mental health
- Three (3) or more years rehabilitation or clinical experience with individuals with brain injuries
- Minimum of 3 years of management or supervisory experience in mental health, mrc or acquired brain injuries.
- Valid driver’s license and ability to drive
- Able to meet and maintain required certification for First Aid, CPR and Medication Administration
- Must possess computer skills necessary to meet the demands of the job, including proficiency in Microsoft Office, E-mail systems (Outlook), and the internet
- Strong interpersonal, written and oral communication skills required
Responsibilities:
- Participate in the supervision and development of KDC’s Acquired Brain Injury programs.
- Provide ongoing supervision, evaluation and support to assigned staff
- Assist with identifying and facilitating staff training and development
- Promote individual growth in the areas of choice and decision-making, community membership, relationships and personal accomplishments
- Supervise the case management process for assigned individuals
- Ensure provision of direct service support in response to needs of individuals
- Assist Program Director in securing all appropriate entitlements/benefits for individual
- Responsible for coverage at sites when necessary
- Provide ongoing monitoring of individual funds
- Facilitate regularly scheduled staff meetings
- Provide individual supervision to staff on a regularly scheduled basis
- Serve as a program liaison to family members/guardians
- Facilitate development and implementation of Individual Support Plans
- Complete and maintain documentation as required
- Monitor and coordinate domestic up-keep of the program site
- Demonstrate an understanding of and commitment to the implementation of agency standards regarding the individuals’ quality of life
- On-call responsibilities as assigned
- Ensure individuals are treated with respect and dignity at all times, and maintain confidentiality regarding their records and other information
- Provide emergency beeper coverage during non-office hours as assigned
- Participate in staff meetings, group and individual supervision and case reviews
- Maintain compliance with all Kennedy-Donovan Center policies and procedures, DDS/MRC regulations, requirements and quality service standards
- Perform other job-related responsibilities as assigned
Physical Requirements:
- Moderate local travel required
- Operation of specialized and adaptive equipment
- Frequent movement, including but not limited to, bending, sitting, lifting and carrying
- Ability to lift 200 lbs with assist or adaptive equipment
