Seeking a Registered Nurse for our growing Adult Family Care program. Responsible to provide oversight and training while working closely with those receiving services, their families, care providers and service partners to develop and enhance truly personalized services and relationships. This position offers flexibility and autonomy while utilizing your nursing assessment, care planning and case management skills to provide services to individuals in home settings. Case Management nursing does not require the delivery of skilled nursing procedures. Requirements for the positions include: Associate’s or bachelor’s degree, RN license, computer abilities, travel/organizational planning, flexibility, excellent communication, training, referral and planning skills. Valid MA driver’s license with clear RMV background check. Must have 2 years recent experience working with adults with disabilities or the elderly.
