Registered Nurse (RN) Job Description:

Reporting to the VP of Quality Assurance, the Residential RN is responsible for ensuring that direct health care and training is provided to residential staff and that services are coordinated with each recipient's primary care physician or medical clinic. The Residential RN provides medical oversight for residential programs and

participates as a member of the interdisciplinary team.

Position Schedule: 32 hour per week, Monday to Friday. May increase to 40 hours in the future.

Essential Functions:

Responsible for the development, implementation and monitoring of Residential nursing oversight, case management and consultative support.

Oversees admissions to residential programs including managing discharge and readmission following acute, rehabilitative or LTC.

Consults to key staff around complex medical issues and the management of home care in the group homes.

Manages Residential Electronic Health Care Records and acts as a point person for State licensing around assigned standards.

Supports Medication Administration implementation and provides training as needed for competency areas such as G-Tube, EPI-Pen and other topics.

Supervises completion of assessments and implementation of health care plans to respond to changes in the medical conditions for the clients we serve.

Communicates with guardians and family members around key medical issues.

Supervises team communication with Physicians and medical centers to ensure all medical information is updated and available at all times.

Directs team assistance, oversight and training for staff to ensure the implementation of services including all needed unique needs training for diagnoses, medications and treatments.

Helps administer monitoring system to ensure physician’s orders are documented and carried out as prescribed.

Enhances and promotes The Arc of Opportunity brand, strategic initiatives and organization’s mission.

Develops and executes short/long term goals which enhance and support organizational strategic plan and priority initiatives.

Provides consultation, case management and site visits as required.

Acts as liaison with local hospitals for admissions, course of hospitalization and discharges as well as general improvement of service delivery/coordination in conjunction with DDS area nurse.

Utilizes the nursing process to ensure that medical trainings are relevant, based on evidence-based practices by using available resources as well as in compliance with DDS and MAP standards.

Appropriately maintains pertinent documentation.

In collaboration with the Residential Manager/Assistant Residential Manager, provides advocates for and monitors specialized and routine medical care to ensure that consumers receive normative quality care, treatment and respect in the community.

Documents each recipient's health status via an annual health/dental assessment quarterly

Ensures compliance with all federal, state and agency policies within assigned residential programs.

Uncomplicated treatments may be performed on occasion.

Other nursing related duties as assigned.

Physical Requirements:

Ability to lift up to 25 lbs.

Ability to kneel, bend and carry items

Ability to sustain periods of walking, standing, sitting

Ability to see, speak and hear

Driving a motor vehicle and/or van, agency and/or personal

Qualifications:

Current licensure to practice as a Registered Nurse by the Commonwealth of Massachusetts Board of Registration of Nursing

A valid driver’s license, acceptable driving record and CORI/Fingerprint checks are required at all times during employment at The Arc of Opportunity.

Proven ability to assist with the development and implementation of in-service programs.

Good working knowledge of and commitment to residential group home setting

Functional and situational reasoning, ability to gather and apply factual information, exercise sound judgment, problem solve and apply principles of influence systems such as motivation, team building, accountability, and leadership with the scope of the position.

Cross-functional communication skills which convey clear direction, desired results and vision to individuals/families served, government agencies, peers and executive management.
