The Arc of Greater Plymouth is seeking a Coordinator for our Shared Living Program. This Coordinator is responsible for coordinating services for a case load of 10-12 Individuals and supervising their Caregivers. They will also assist these Individuals achieve goals with on-going support and advocacy through the ISP Team and person centered planning. They will ensure all DDS regulations are being met and followed.
Primary Duties and Responsibilities
•Ensure the appropriateness and coordination of housing, vocational, medical, clinical and financial supports for program participants
•Assist the Program Director with planning, implementing and evaluating the Supported Living Program . .
•Attend participants “Team Meetings” to take minutes. Such attendance to “Team Meetings” is vital to stay in the loop to the progress/regression on the Individuals served, and to better assist in the decision-making process for them.
•Complete the respite scheduling in conjunction with the SL Caregivers. This entails extensive communication with Caregivers, Respite Relief Staff, and other agency Directors.
•Ensure that ISP goals are implemented by Community Companions & Caregivers.
•Input ISP goals and assessments in HCSIS for the programs participants.
•Tracking of ISP deadlines and dates, while in some instances, attending ISP meetings;
•Supervise Caregivers and ensure the safety and security of the family care home by monthly home visits, monitoring financial documentation, developing safety plans and maintaining caregiver files.
•Filing and processing of approved payment authorizations for Caregivers, Respite & Companions.
•Maintain budget tracking for Individuals
•Ensure the Case Management process is completed on all participants in the Shared Living Program.

•Excellent verbal and written communication skills
•Work cooperatively and effectively with others to set goals, resolve problems, and make decisions that enhance organizational effectiveness.
•Proven organizational and effective time management skills
•Assess problem situations to identify causes, gather and process relevant information, generate possible solutions, and make recommendations and/or resolve the problem.
•Proven success developing positive working relationships with diverse stakeholders.
•Bachelor’s degree in Human Services preferred
•Knowledge of program management and of client groups
•Must be able to pass criminal background check
