Adult Health Services Supervisor - The Guild for Human Services - Concord, MA

Job Title: Adult Health Services Supervisor

Job Category: Supervisor: Exempt/UFR 108

Reports to: Director of Health Services

Time required: Full-time or part-time, evening/weekend hours required, plus minimum of one week of call per month

Supervises: Adult Nurse Case Managers

Summary: Coordinate medical care for the individuals served and facilitate communication with outside agencies and physicians as necessary.

Primary Job Responsibilities:

  • Meet with direct reports at minimum two times per month and document supervision notes.
  • Monitor direct reports performance and complete yearly evaluation process
  • Assign caseload and responsibilities to direct reporting nurses
  • Facilitate the health care planning process in conjunction with Residential Manager for the transitioning of individuals into Guild adult residences
  • Provide staff training to ensure optimal health and safety standards
  • Monitor the health and safety issues within all adult residences
  • Maintain medical records including documentation of treatment and medication administration of individuals
  • Complete initial and annual medical assessments of individuals
  • Active member of the ISP team
  • Participate in designated team meetings and case reviews for individuals
  • Communicate with outside agencies, physicians and guardians.
  • Monitor MAP in all adult group homes for quality assurance
  • Report deficient practice by Certified MAP staff in accordance with MAP Policies
  • Provide medical on-call coverage at minimum one week monthly
  • Able to attend training as required
  • Perform other related duties as assigned

Essential Job Functions

  • Regular attendance at work is an essential function of the job including inclement weather situations.
  • Physical and mental capacity to work in stressful situations and de-escalate individuals who have limited cognition and complex needs
  • Proficiency in written and spoken English is an essential function of the job.
  • Excellent organization and time management skills.
  • Pass Guild Driving test to access Guild vehicles for transportations of individuals.
  • Must be able to implement crisis intervention techniques as necessary, including Nonviolent
  • Crisis Intervention (via certification through CPI)

Qualifications:

  • Registered Nurses who have an associate’s degree (bachelor’s preferred) with 5+ years Nursing experience.
  • Knowledge of Massachusetts regulatory standards is required with preferred experience in regulatory compliance with standards from Department of Developmental Services (DDS) and Department of Public Health (DPH)
  • Must meet Massachusetts Criminal Offender Record Information (CORI) background check requirements
  • Proficiency in Microsoft Office Suite
