Community Inclusion Specialist - Delta Projects, Inc. - Dedham, MA

Delta Projects is hiring for a Community Inclusion Specialist.

A Community Inclusion Specialist will provide supported employment services to adults with developmental disabilities who wish to be a member of the workforce. Employment settings include in-house job readiness programs and various community-based employment settings. This position will also promote new learning skills, and help individuals attain the maximum level of independence.

ESSENTIAL FUNCTIONS:

Assist individuals with the development of appropriate job sustaining behaviors, task analysis, and skills to complete specific job tasks, natural supports, and the identification and implementation of reasonable accommodations at the job site.
Assist individuals in exploring interests and developing new skills by teaching classes, leading community resource tours and encouraging exploration.
Assist participants to identify intrinsic and extrinsic motivators in regard to personal and vocational interests and use information identified for inclusion opportunities in competitive and volunteer markets.
Meets with participants to identify barriers to employment and develop strategies to manage these issues.
Maintains complete, accurate and timely case notes and related records in manual and or electronic systems.
Assist in the development of an individualized success plan for each participant.
Implement the success plans and provide necessary supports to aid participants in achieving measurable objectives.
Collect data and maintain records as required by the program; maintain records regarding employment history, progress, and learning styles including case notes and time logs.
Be available to answer questions and provide feedback for the participants.
Provide ongoing supports as needed to facilitate employment retention.
Utilize job carving and job creation strategies as needed.
Use information about learning style, emotional and behavioral issues, health status and other relevant issues to assist in the development and implementation of service delivery strategies that reflect the needs and preference of each participant.
Function as an ambassador for Delta Projects, Inc. within the community
Provide case management and service coordination to facilitate access to and coordination of services necessary for successful employment outcomes.
Serve as the Human Rights Officer for individuals on assigned caseload advising individuals on their rights and responsibilities.
Any additional duties as assigned by supervisor.

QUALIFICATIONS:

Minimum of a High School Diploma or equivalent; Associate’s Degree preferred; or at least one year job development experience and/or training, or equivalent combination of education and experience
Current and valid driver’s license in the state of residence
Must possess a safe, reliable, registered, insured, and inspected vehicle in order to transport individuals as needed/required
Qualifying criminal background and driving record
Must be able to demonstrate competence in certain areas of training including but not limited to CPR, First Aid, non-violent physical interventions, behavior management, medication administration, and driving
Professional oral and written communication skills; function as an ambassador for Delta Projects, Inc. within the community and maintain a professional presentation in physical appearance and communication style.
Must be willing to work in excess of 40 hours per week as driven by operational needs
Knowledge of funding source regulatory requirements and policies/procedures
Must be able to attend, participate, and pass employment courses provided by the Institute for Community Inclusion
Proficiency with Microsoft Office
