Community Supports Coordinator Job Description
Responsible for coordination of community based individualized support opportunities for individuals with disabilities as well as providing meaningful activities that enhance community related skills.
Position Schedule: Tuesday and Wednesday (12pm to 7pm), Thursday and Friday (12pm to 8pm), Saturday (9am - 5pm) Some flexibility with schedule.
Location: Position will travel in Fitchburg, Worcester, and North Central Mass.
Essential Functions:
Works closely with Individualized Supports Case Managers and Division Director in delivering service standards of excellence and ensuring operational compliance within assigned program in accordance with federal, state and agency regulations
Implement Individual Support Plans for individuals based on assessments, objectives and goals
Create weekly and monthly skill development schedules based upon participants ISP goals, skills, and preferences
Develop a home and/or community curriculum
Complete and submit required electronic documentation, which includes monthly attendance, billing and production sheets
Timely and accurate coding for services into electronic medical records system for billing purposes
Ability to access all modes of transportation including driving and public transportation
Attend mandatory trainings and meetings as required and assigned
Actively advocates for participants
Assists individuals with skill building and attainment of ISP/service plan goals
Provides transportation to participants to community activities including performing safe lifts and transfers
Demonstrates commitment to agency mission and The Way of The Arc in all interactions
Other duties as assigned
Required Qualifications:
High school diploma or GED is required.
A valid driver’s license and acceptable driving record and CORI is required at all times during employment with The Arc of Opportunity.
Computer literacy skills.
Physical Requirements:
Ability to lift up to 50 lbs., kneel, bend and carry items, and sustain periods of walking, standing, and sitting
Ability to physically assist individuals according to their needs and perform physical restraint if necessary
Community Supports Aide Mental Requirements:
Cross functional communication skills in written and oral English which convey clear and concise information in a professional and respectful manner to individuals supported, family members/guardians, peers, government agencies, community partners and employees
Functional and situational reasoning to follow policy, problem solve, exercise sound judgment, prioritize multiple tasks, and manage crisis
