GENERAL SUMMARY

Provides the Lead support and assistance to program participants in the areas of independent living, self-management within the community, social and independent living skills development and affective and behavioral management. Communicates with families and providers, as required.

ESSENTIAL JOB FUNCTIONS*

  • Establishes and maintains a rapport with day habilitation program participants. Ensures and safeguards members’ human dignity, rights and liberties.
  • Plans and oversees participant services in accordance with day habilitation regulations in the areas of independent living, self-management within the community, social and independent living skills development and affective and behavioral management.
  • Assists program participants in making and expressing choices based on assessments and other service need indicators. Creates goals and objectives. Plans program activities in accordance with those goals and supervises implementation of services created by interdisciplinary team (IDT).
  • Liaison to program case managers to ensure documentation compliance and in keeping with the development of the Day Hab Service Plan (DHSP) goals.
  • Review case management with team in assigned area to ensure implementation of goals.
  • Attends, or assigns DHSM, team and ISP meetings for participants in the assigned area.
  • Lead Participant group meetings to develop individual and group activity choices in activities and schedules.
  • Meets on a regular basis with the individual participant to review personal goals and makes recommendations on changes through Day Habilitation case managers as necessary.
  • Assists in the development and expansion of opportunities for community integration based on the needs, choice and wants of the program participants in keeping with Day Habilitation scope of service.
  • Provides physical assistance to program participants, as needed, including, but not limited to, toileting, transferring and feeding.
  • Adheres to all funding source regulations and Agency policies and procedures.
  • Keeps management abreast of planned activities, as well as, any potential participant issues or needs, which are not being met.
  • Establishes and maintains professional relationships with colleagues, management and community partners.
  • Serves as mandated reporter for suspected cases of abuse and/or neglect to the Disabled Persons Protection Commission (DPPC).
  • In addition, may perform one or more of the following functions:
  • Fosters good working relationships with community partners and other organizations.
  • Writes and develops participant progress notes and assessments, as well as, compiles daily, weekly, and monthly reports.
  • Fosters maximum independence while decreasing support level in individual’s plan.
  • Present as a positive role model for participants and model professional behavior for staff.

PREPARATION, KNOWLEDGE, SKILLS & ABILITIES

  • Bachelor’s degree preferred.
  • Minimum 2 years prior experience in a community health setting.

SUPERVISORY RESPONSIBILITY

  • Maintains a weekly activity planner for the assigned program created by program staff and participants. Submits planner to the Day Habilitation Director for review and approval.
  • Implements and assigns planned activities for program area staff to engage participants in a structured and scheduled, meaningful day in accordance with Day Habilitation scope of service.
  • Assign case management and documentation to Day Habilitation Service Managers in program area and ensure accuracy and timely completion of that documentation.
  • Lead area program staff planning meetings with focus on activity planning, participant goal development, strategies, and problem solving and goal achievement.

WORKING CONDITIONS/PHYSICAL DEMANDS

  • Frequent standing, walking, bending, and twisting.
  • Ability to lift up to 50 pounds.
  • Ability to pull up or transfer individuals weighing up to 250 pounds.
