Position Title: Health Care Supervisor/RN

Program: Day Habilitation

Reports To: Director of Day Habilitation

FLSA Status: Exempt

Salary: $85,000

Job Overview: Coordinate, monitor and support the healthcare and physical needs of people served. Responsible for monitoring the medication system. Provide direct nursing care and staff training. Ensure that services provided are in compliance with Mass Health, DDS, ICARF, DPH and agency policy and procedures.

Supervisory Responsibilities:

Supervises: Licensed Practical Nurse (LPN)

Essential Functions of Position:

1. Coordinate and oversee medical services provided to people served.

2. Follow regulations in accordance to Mass Health, DPH, CARF and DDS guidelines.

3. Monitor and report medical progress, challenges and status for people served.

4. Ability to work collaboratively as a member of theinterdisciplinary treatment team and attend scheduled meetings.

5. Oversight of medical devices such as lifting devices,wheelchairs, standers.

6. Provide oversight of treatment as recommended by a licensed therapist and Primary Care Physician(PCP) orders.

7. Participate in the recruitment and hiring process of staff.

8. Provide on-going supervision and job performance evaluation for the LPN.

9. Ensure that all medication and drugs are stored and secured in accordance with regulatory guidelines.

10. Assist and attend to personal care needs/ADL’s for people served as needed.

11. Provide physical support, assistance with lifting and transferring people served as needed.

12. Review and ensure that both electronic and paper medical records such as annual updates of physical examinations, emergency treatment forms, doctors’ orders are current and meet established guidelines.

13. Attend all required trainings and maintain current certifications/licensure.

14. Participate in completing severity profiles, establishmedical plans of care, and document monthly and semi-annual progress, participate in program referrals and discharges process.

15. Maintain an inventory of medical supplies and place orders in accordance with regulatory guidelines and agencypolicies and procedures.

16. Advocate for access to high quality health care for people served and facilitate communication with medical professionals.

17. Utilize the healthcare record system to update information as required and complete all required documentation in a timely manner.

18. Supervise daily operations of the Day Hab program in the absence of Program Director and Day Hab service Manager.

19. Provide assistance to other programs as needed.

20. Participate in Day Hab Service Plan (DHSP) development and meetings.

21. Monitor Day Hab lunch as assigned and ensure that individual adaptive equipment, protocols and diets are followed.

22. Perform other work related duties as assigned by supervisor.

EDUCATION and/or EXPERIENCE

Current Massachusetts license to practice as a Registered Nurseand at least 2 years of nursing experience. Prior supervisory experience preferred. Experience working with individuals with Intellectual/Developmental Disabilities preferred. Knowledge of rehabilitation activities, Mass Health, CARF and DDS regulations desired. Demonstrated ability to handle multiple responsibilities in a fast-paced and challenging environment.

LANGUAGE SKILLS

Ability to read, analyze, and interpret general program information: instructions, implementation procedures, and agency regulations. Write clear reports, data collection, and progress reports. Effectively present information and respond to questions from groups of managers, clients, customers, and the general public.

MATHEMATICAL SKILLS

Work with mathematical concepts such as probability and statistical inference. Apply concepts such as fractions, percentages, ratios, and proportions to practical situations.

REASONING ABILITY

Ability to define problems: collect data, establish facts, and draw valid conclusions.

CERTIFICATES, LICENSES, REGISTRATIONS

Maintain a current license to practice nursing in Massachusetts as a Registered Nurse, Requires a valid driver’s license and acceptable driving record. MAP trainer certification. Maintain required trainings and Personal Liability Insurance current. Pass a Criminal Background Check (CORI).

PHYSICAL DEMANDS

While performing the duties of this job, the employee is frequently required to stand; walk; use hands to finger, handle, or feel; and reach with hands and arms. The employee is occasionally required to sit; climb or balance; stoop, kneel, crouch, or crawl; and talk or hear. Ability to lift 50 pounds as needed.

WORK ENVIRONMENT:

While performing the duties of this job, the employee is occasionally exposed to wet and/or humid conditions, moving mechanical parts, outside weather conditions, and vibration. The noise level in the work environment is usually moderate. The work environment characteristic will change depending on the location of services and supports being offered to the individuals.
