The Licensed Practical Nurse provides nursing services to individuals within Day Services, under the oversight of the Healthcare Coordinators. This position will work closely with the Interdisciplinary team to ensure the health and wellbeing of all individuals through routine assessment, hands on care and training.

Responsibilities

Monitor all nursing services for all individuals with correspondence through primary care physicians/nurse practitioners, residential managers, etc.

Monitor recommendations in conjunction with the comprehensive written therapeutic service plan (DHSP) process through the interdisciplinary team.

A key member of the Day Habilitation Interdisciplinary team.

Provide detailed semi annual reviews on all aspects of individuals’ day habilitation programming, reviewed by the Healthcare Supervisor.

Provide daily nursing notes and monthly summaries in accordance to regulations, expectations and deadlines.

Evaluate all individuals in day habilitation annually providing a detailed plan of care, reviewed by the Healthcare Supervisor.

Provide training and support to staff within day services.

Provide consultation to individuals within Community and Employment services.

Review, maintain and update medical information in each file.

Administer prescribed medication as necessary, including oral medication and injections.

Educate and assist staff with nursing feeding requirements for all individuals when necessary or medically indicated.

Provide daily nursing care to individuals (ie. colostomy care, G-tube, catheter, oxygen treatments, etc).

Assist with community inclusion for those requiring nursing oversight and care.

Promote self-advocacy and offer consultation to individuals as needed.

Maintain all licenses/certifications current in accordance to State of MA in nursing

Attend trainings as assigned; maintains necessary certifications (CPR, MA RN etc).

Strictly adhere to all HIPPA regulations and maintain confidentiality at all times

Provide emergency first aid and provide direction to staff in the case of a medical crisis.

Develop and train all staff regarding medical protocols (i.e. medication, diagnosis/disability, monitoring health status following restraint, exercise or other strenuous activities) as necessary and ensure the implementation and follow-through.

Provide structured opportunities for groups and individuals to ask questions regarding health/medical information.

Communicate with providers and caregivers as needed (ie. medication refills, doctors’ orders, required medical paperwork, etc).

Consult with all day services team members and other essential individuals on a day-to-day basis to share any pertinent medical information.

Work collaboratively with Healthcare Coordinators.

Maintain open and clear communication about all daily responsibilities with Healthcare Coordinators.

Assist in the training of staff regarding nursing protocols (ie. medication, diagnosis/disability, etc) as necessary. Ensure the implementation and follow through.

Qualifications

High school diploma or associates degree with a valid Massachusetts practical nursing license and 1 year experience in a community healthcare setting.

One year of experience in a community health care setting.

Must be able to perform each essential duty satisfactorily.

Strongly prefer that a candidate will have a demonstrated understanding of and competence in serving culturally diverse populations.

Strong oral and written communication skills with individuals, families, funding sources, consultants, the community at large, and the program and administrative staff.

Must hold a valid drivers’ license. Must have access to an operational and insured vehicle and be willing to use it to transport consumers.
