Supervise and manage the provision of home-based supports and community resources for individuals with developmental disabilities, utilizing a team-based approach. Promote individual growth in the areas of choice and decision-making, community membership, relationships and personal accomplishments.
Responsibilities:
- Provide ongoing supervision, evaluation and support to assigned staff.
- In collaboration with Regional Director, function as a liaison with funding agencies.
- In collaboration with Regional Director, provide leadership role in program development.
- In collaboration with Regional Director, identify and facilitate staff training and development.
- Provide leadership role in promoting staff commitment and implementation of quality service principles.
- In collaboration with Regional Director, provide orientation, ongoing supervision and support of assigned staff.
- Supervise the case management process for assigned individuals.
- Facilitate and empower an individualized approach to all aspects of support planning, delivery, and evaluation, with a focus on quality of life outcomes.
- Ensure provision of direct service support in response to needs of individuals.
- Assist Regional Director in securing all appropriate entitlements/benefits for individuals.
- Provide ongoing monitoring of consumer funds.
- Ensure that each individual’s healthcare needs are met.
- Facilitate regularly scheduled staff meetings.
- Provide individual supervision to staff on a regularly scheduled basis.
- Serve as a program liaison to family members/guardians.
- Facilitate development and implementation of Individual Support Plan.
- Ensure implementation of all necessary clinical supports.
- Complete and maintain documentation as required.
- Ensure the legal, civil, and human rights of individuals supported.
- Provide emergency beeper coverage during non-office hours as assigned.
- Participate in staff meetings, group and individual supervision and case reviews.
- Maintain compliance with all Kennedy-Donovan Center policies and procedures, DDS regulations, requirements and quality service standards.
- Demonstrate commitment and enthusiasm in supporting individuals in creative and innovative ways.
- Ensure individuals are treated with respect and dignity at all times, and maintain confidentiality regarding their records and other information.
- Participate in professional development activities and training necessary to enhance skills and maintain certification.
- Perform other job-related responsibilities as assigned.
Physical Requirements:
- Moderate travel required.
- Operation of specialized and adaptive equipment.
- Frequent movement within home and community setting included but not limited to bending, sitting, lifting and carrying.
- Ability to use computer.
- Ability to lift 100lbs.
Qualifications:
- Bachelor’s degree required.
- Experience with individuals with special needs.
- Strong interpersonal and communication skills.
- Administrative and/or supervisory experience.
- Commitment to community based supports for individuals with developmental disabilities.
- Valid driver’s license.
- Able to meet required certification for First Aid, CPR and Medication Administration.
- Able to meet required competency level in Microsoft Office Software.
